What does a waterproof rating like 5000mm or 12000mm actually mean?

It's the hydrostatic head measurement: how tall a column of water the fabric resists before leaking. Higher numbers indicate more resistance to sustained downpours, though these figures come from manufacturers and weren't independently verified in the data we reviewed. For most day hikes anything in the 5000mm-plus range is adequate.
